We require a minimum amount of information to approve reported cases for investigation.  Please ensure that you review the following checklist and have the necessary information before continuing:

• You are NOT reporting WELFARE Fraud.  To report Welfare, please call (800)349-9970.
• You are reporting fraud that involves a Los Angeles County employee or a contractor or vendor doing business with Los Angeles County.
• You know who the suspect(s) involved is.
• You know what the alleged fraud incident is.
• You know how the alleged fraud incident was committed.
• You know where the alleged fraud incident was committed.
• You know when (or approximately) the alleged fraud incident was committed.
